无创通气在COPD并呼吸衰竭患者的临床应用研究

Clinical ivesigation of non-invasive positive pressure ventilation in COPD patients with respiratory failure

目的:探讨无创通气在慢性阻塞性肺疾病(COPD)伴呼吸衰竭患者中的应用价值。方法:选择30例需要呼吸支持的COPD伴呼吸衰竭患者进行无创通气,采用以自主呼吸触发为主的通气模式(PSV、SIMV、SIMV±PSV),在上机前和上机后1、2、6、12小时测定动脉血气分析,进行统计学分析。结果:上机后各组血气指标pH、PaO2、PaCO2与治疗前相比,差异有显著性。结论:无创通气可以明显减少COPD呼吸衰竭患者的插管机率,降低呼吸机相关肺炎的发生率。

Objective ：To estimate the efficacy of non-invasive positive pressure ventilation （NIPPV） in chronic obstuetiv pulmonary disease（COPD） with respiratory failure.Methods： Self-trigger modes （PSV, SIMV, SIMV±PSV） were adopted to 30 COPD patients with respiratory failure who needed respiratory support.Arterial blood gas analysis was tested before ventilation and after ventilation 1h,2h,6h, 12h.Stafisties was analysed.Results：The difference of blood gas index before and after ventilation was significant. Conclusion：NIPPV can greatly decrease the tracheal intubation chance of COPD patients with respiratory failure and lower the chance of VAP.
