慢性乙型肝炎、肝硬化患者血清瘦素的测定及其与胰岛素水平的相关性研究

Leptin level in patients with chronic hepatitis B and cirrhosis and its correlation with fasting insulin level

目的探讨慢性乙型肝炎和乙型肝炎后肝硬化患者血清瘦素水平和空腹胰岛素水平并探讨两者间的关系.方法43例为慢性乙型肝炎患者(乙肝组),41例为乙型肝炎后患者,38例为正常对照组,分别测定血清瘦素水平、空腹血糖和空腹胰岛素浓度.结果女性瘦素水平高于男性(P<0.01);乙肝组和肝硬化组患者瘦素水平高于对照组(P<0.01);乙肝组和肝硬化组血清胰岛素水平和胰岛素抵抗指数高于对照组(P<0.01);各组患者的血清瘦素水平和空腹胰岛素、胰岛素抵抗指数呈正相关(P<0.01).结论慢性乙型肝炎和肝硬化患者的血清瘦素水平明显升高,提示可能跟病毒感染、肝功能损害有一定的关系.

AIM： To investigate the leptin level, the fasting insulin level and their correlation in chronic hepatitis B patients and cirrhosis patients. METHODS： The leptin level, the fasting insulin level and fasting glucose level were detected in the hepatitis B group （ n = 43 ）, the cirrhosis group （ n = 41 ） and the normal group（ n = 38）. RESULTS： The leptin levels in women were higher than those in men（ P 〈0.01 ）. The leptin levels in hepatitis B group and cirrhosis group were higher than those in the normal group（P 〈0.01 ）. The fasting insulin levels and the insulin resistance index in hepatitis B group and cirrhosis group were higher than those in the normal group （ P 〈 0.01 ）. There were positive correlations among the leptin level and the fasting insulin level and insulin resistance index. CONCLUSION： The leptin levels in hepatitis B patients and cirrhosis patients are increased, and this is maybe related to the virus infection and the liver function impairment.
