Ki-67在涎腺腺样囊性癌组织中的表达及其与预后相关性的探讨

Expression of Ki-67 in salivary adenoid cystic carcinoma and Its relationship with prognosis

目的探讨增殖细胞核抗原标记物(Ki-67)在涎腺腺样囊性癌(SACC)组织中的表达情况及其与预后的相关性。方法采用免疫组织化学S-P法测定Ki-67在54例SACC组织中的表达,其中术后复发组19例,未复发组35例;死亡组16例,生存组38例。采用Cox比例风险回归模型对影响SACC患者术后生存的多项因素进行分析。结果死亡组Ki-67的表达明显高于生存组(P=0.004);在术后复发组的表达高于未复发组(P=0.017)。Cox模型多因素分析结果显示,Ki-67、TNM分期、病理类型和术后复发是影响SACC预后的独立指标,危险度分别为3.236、1.382、0.213、2.316。结论Ki-67在SACC组织中的表达与其临床预后有关,Ki-67的高表达预示着SACC的预后较差,探讨Ki-67的表达情况有助于SACC临床治疗方案的制定。

Objective To investigate the expression of Ki-67 in salivary adenoid cystic carcinoma（SACC） and its relationship with prognosis. Methods The expression of Ki-67 in SACC of 54 cases were studied by immunohistochemical staining, of which 19 cases recurred（ recurrent group） ,35 cases non-recurred（ non-recurrent group） , 16 cases died（ deceased group） and 38 cases survived（ survival group）. Cox regression model was used for multivariate prognostic analysis. Results The expression of Ki-67 in deceased group were higher than those in survival group（P =0. 004）. Overexpression of Ki-67 could be found in recurrent group than those in non-recurrent group（ P = 0. 017）. On Cox regression of multivariate analysis, Ki-67 ,TNM stage, pathological type, and local recurrence were shown to be independent prognosis factors for survival of SACC. RR were 3. 236,1. 382,0. 213,2. 316 ,respectively. Conclusion Ki-67 was related to the prognosis of SACC, overexpression of Ki-67 indicated poor prognosis of the disease, and it might be helpful for the treatment of SACC.
