胸腰段椎间盘突出症的手术疗效分析

Thoracolumbar disc hernination treated with posterior transpedicular decompression

目的评价采用后路经椎弓根脊髓减压术治疗胸腰段椎间盘突出症的疗效。方法对24例胸腰段椎间盘突出症患者采用后路经椎弓根脊髓减压。患者均行X线、CT及MR检查。MRI显示本组24例中合并黄韧带骨化(OLF)9例,后纵韧带钙化(OPLL)10例。椎间隙相应水平硬膜囊或脊髓受压。平均随访1～2.5年,用Otanni评分系统进行疗效评估。结果Otanni评分,优11例,良9例,可3例,差1例,优良率达83.33%。无神经症状加重等严重并发症,1例术后出现脑脊液漏,保守治疗治愈。结论后路经椎弓根脊髓全周减压治疗胸腰段椎间盘突出症临床效果良好,并能减少并发症。

Objective To evaluate the clinical results and the value of usefulness with the operation of posterior transpedicular decompression in treating thoracolumbar disc hernination. Methods 24 patients who suffered from thoracolumbar disc hernination were treated and followed up from 1 to 2.5 years. All patients underwent X-ray,CT and MRI examination. Using Otanni scored system to evaluate the clinical results. Results A perfect result was found in 11 patients,a good result was found in further 9 patients,a fair satisfactory rate was 83.33%. No nervous symptoms were found. Conclusion The posterior transpedicular decompression shows a good result in treating patients who suffered from thoracolumbar disc hernination.
