不同年龄患者人工心脏起搏术后的临床效果及并发症比较

The comparison of clinical effect and complication after artificial cardiac pacing operation in different ages patients

目的比较不同年龄患者使用永久性心脏起搏器的治疗效果和并发症的发生情况。方法285例缓慢性心律失常患者根据年龄被分为老年组146例和非老年组139例,均在心导管室内无菌操作下安装永久起搏器,所有患者均分别于出院后1、3、6、12个月,以后每年1次接受临床症状随访。结果两组患者均植入成功。老年组和非老年组患者的3年生存率分别为92．5%和93．5%,两组间生存率差异无统计学意义（P＞0．05）;老年组和非老年组患者并发症的发生率分别为10．3%和5．0%,两组间差异有统计学意义（P＜0．05）。老年组和非老年组与手术有关的并发症主要为囊袋出血、囊袋感染,与起搏器及电极有关的并发症包括电极移位、起搏器电池提前耗竭、局部肌肉刺激等,与起搏方式有关的并发症主要为起搏器综合征。结论老年人和非老年人安装人工起搏器后的临床疗效相近,但老年人并发症发生率较高。

Objective To compare therapeutic efficacy and complication happening of different ages patients using permanence artifici＇al cardiac pacemaker in different ages patients. Methods 285 cases slow cardiac arrhythmia patients were divided into older age group（ n = 146） and non-older age group（ n = 139） according to age. All patients were fixed permanence artificial cardiac pacemaker conclusion in the condition of cardiac catheter room sterile operation,and all patients accepted clinical symptoms follow-up visit in 1,3,6,12 month and 1 every year afterward. Resuits In two groups all patients were successfully implanted. 3 years survival rates of the older group and non-older group differently were 92.5 % and 93.5 % （P 〉 0.05 ）, and complication happening rates differently were 10.3 % and 5.0 % （P〈0.05 ）. Complications connecting with operation mainly were sac hemorrhage and infection, and connecting with pacemaker and electrode included electrode shift and battery advance exhaust and local muscle stimulation and so on; connecting with mode of cardiac pacing were pacemaker syndrome. Conclusion Clinical effect using permanence artificial Cardiac pacemaker is close to each other in older age and non-older patients, but older patients have a higher complication happening rate.
