变应性鼻炎患者血清特异性IgE分析

An Analysis of Serum Specific IgE in Patients with Allergic Rhinitis

目的观察变应性鼻炎(allergic rhinitis,AR)患者46种常见变应原血清特异性 IgE(SIgE)的阳性检出率,以及合并有哮喘和(或)特应性皮炎等个人过敏史对其阳性检出率的影响。方法应用 UniCAP-100E 免疫荧光定量分析系统检测80例 AR 患者 SIgE。结果 AR 患者粉尘螨、屋尘螨的 SIgE 阳性检出率分别为68.75%,67.50%;粉尘、屋尘检出率分别为41.25%,38.75%;昆虫类中飞蛾、蟑螂、蜂毒检出率分别为47.50%,38.75%,20.00%;垫料类中蚕丝、棉花检出率分别为31.25%,15.00%;花粉以苋科为主,检出率为18.75%;真菌类以交链孢霉、白色念珠菌、蠕孢霉、烟曲霉为主,检出率分别为13.75%,13.75%,12.50%,10.00%;猫、狗毛的检出率分别为12.50%,8.75%;食物以虾、蟹为主,检出率分别为38.75%,36.25%。合并哮喘和(或)特应性皮炎等个人过敏史的 AR 患者 SIgE 阳性检出率与无此病史的 AR 患者差异有统计学意义(P<0.01)。结论螨、灰尘、昆虫、垫料、花粉、猫狗皮毛、部分真菌及食物虾、蟹是 AR 常见变应原;合并有哮喘和(或)特应性皮炎等个人过敏史 AR 患者 SIgE 阳性检出率明显高于无个人过敏史患者。(中国眼耳鼻喉科杂志,2007,7:153-154)

Purpose To observe the positive rate of specific IgE （SIgE） of 46 kinds of common allergens in serum of patients with allergic rhinitis （AR） and to compare the positive rate between patients with and without history of asthma and atopic dermatitis（AD）. Methods Eighty cases with AR were collected in this study. The serum SIgE was measured with quantified UniCAP-100E system. Results The positive rate of SIgE of dermatophagoides culinae and dermatophagoides pteronyssinus was 68.75% and 67.50% respectively; dust and house dust was 41.25% and 38.75% ; and moth, cockroach, mellitoxin, padding natural silk and cotton was 47.50%, 38.75% , 20.00%, 31.25% and 15.00%. The positive rate of pollen（ amaranthcea in primary） was 18.75%. The detected fungus mainly including blastomyces albicans, altemaria allcmata, helminthosporium halodes, and aspergillus fumgatits whose positive rate of SIgE was 13.75%, 13.75%, 12.50%, 10. 00% respectively. The positive rate of fur of cat and dog was 12.50% and 8.75%. The main food allergens are shrimp and crab, and their positive rate was 38.75% and 36. 25%. There was significant difference （P 〈0.01）in the positive rate of SIgE between AR paitents with and without history of asthma and/or AD. Conclusion The common allergens include mite, dust, insects, padding, pollen, fur of cat and dog and some fungus. The patients with history of asthma and/or AD show higher positive rate of SIgE than those without the history.
