摘要
目的探讨中年患者近视散光中总合散光(total astigmatism,TA)、角膜散光(corneal astigmatism,CA)和晶状体散光(lenticular astigmatism,LA)的分布和相关性。方法对49例(65眼)中年患者进行综合验光和角膜地形图检查,采用 Thibos 矢量法分析近视散光,将屈光度转换成 M,J_0和J_(45)。结果高度散光和低度散光组及循规性散光(with the rule,WTR)和斜向散光(oblique astigmatism,OBL)组的 TJ_0,CJ_0,LJ_0之间和TJ_(45),CJ_(45),LJ_(45)之间的相关性有统计学意义,而逆规性散光(against the rule,ATR)组中只有 TJ_0,CJ_0,LJ_0之间的相关性有统计学意义。WTR 组和 ATR 组的高度散光和低度散光之间,TJ_0,CJ_0,LJ_0的差别有统计学意义。结论随着年龄的增大,晶状体硬化,TA,CA 和 LA 分布有变化,LA 有补偿 CA 的作用,且晶状体斜向散光成分增加。(中国眼耳鼻喉科杂志,2007,7:169-171)
Purpose To investigate the contribution and correlation of corneal and lenticular components to total astigmatism in middle-aged patients. Methods Sixty-five eyes of fourty-nine middle-aged patients were examined with phoropter and corneal topography. The diopters of total and corneal cylinder were transposed into J0 and J45 components with the method of Thibos. Results J0 and J45 components of corneal and lenticular-astigmatism were highly correlated with total astigmatism in highand low-astigmatism group, also in with-the-rule and oblique-astigmatism group, whereas only J0 was significantly correlated with total astigmatism in against-the-rule astigmatism group. The magnitude of J0 component was significantly different between high- and low-astigmatism group in both with- and against-the-rule astigmatism groups. Conclusion Astigmatism component varies in middle-aged patients as aging of lens. The lens has compensation for corneal astigmatism. The lens also increases the oblique astigmatism component.
