鼻翼软骨肌肉环在单侧重度唇裂鼻畸形矫正术中的作用

Alar cartilage-muscle rings in repairing the unilateral severe cleft lip

目的：探讨鼻翼软骨肌肉环在单侧重度唇裂术后鼻畸形整复中的作用。方法：经鼻小柱基底的“V-Y”型切口行患侧鼻翼软骨肌肉环修复，对裂侧上颌骨退缩明显者，在裂侧梨状孔边缘处充填自体肋软骨组织，整复76例唇裂鼻畸形患者，观察近、远期效果。结果：76例切口均一期愈合，随访12～38个月，对唇裂鼻畸形患者的鼻尖、鼻翼、鼻孔及鼻小柱畸形矫治术后外形满意。结论：唇裂患者因鼻翼软骨肌肉环发育不良，进而导致了鼻部肌力的失衡。因此，唇裂鼻畸形矫正中修复鼻翼软骨环肌肉状结构具有重要意义。

Objective： To discuss an effective method of cheiloplasty for postoperative nasal deformity after unilateral severe cleft lip. Methods： Seventy-six patients with secondary cleft lip nasal deformity were repaired with ajar cartilage-muscle rings by remov aj of the points of theattachment of the nasal columella basement through a ＂V-Y＂ incision andimplant ation of an autologous rib cartilage tissue around the piriform apertta-e The short-and long-term responses were observed. Results： All got primary healings. Followed up for 12-38 months, the apex of nose, nasal ala, nostrils and nasal columella produced satisfied outlines. Conclusions： Alar cartilage-muscle ring is hypoplastic, which results in nasal myosthenic disequilibrium. Reconstruction of the ajar cartilage-muscle ring plays a key role in the correction of nasal deformities for cleft lip patients.
