老年膝骨关节炎社区保守综合防治报告

Report of non-operation comprehensive intervention on the elderly knee osteoarthritis in urban communities

目的比较常规治疗与综合防治社区老年骨关节炎(OA)的疗效差异。方法将160例OA患者随机分为两组,常规组采用常规保守治疗手段进行治疗,综合防治组除了常规组采用的手段外,利用大医院专科及社区医疗的优势,开展多种形式的健康教育、电话与上门随访指导、关节合理支具应用等综合手段。结果两组治疗前后Lysholm膝关节评分值比较,综合防治组治疗后分值明显高于常规组(t=2.83,P<0.01),医疗费无明显增加(t=1.02,P>0.05)。结论社区保守综合防治老年膝骨关节炎疗效明显,既发挥了大医院的专科特长,也体现了社区医疗的优势。

Objective To compare the effects of non- operation comprehensive intervention and normal treatment on the elderly knee osteoarthritis in urban communities. Methods 160 cases were randomly into non - operation comprehensive intervention group （ n = 80） and normal treatment group （ n = 80 ）. The patients in normal treatment group received non - operation therapies while the patients in comprehensive intervention group received not only the normal methods but also health promotion education in many ways, special advices according to patients situation by visiting patients or monitoring through telephone closely, physical activities were befittingly suggested, and so on. Results The evaluated score of ＂Lysholm＂ in knee were mostly higher in comprehensive intervention group than the normal treatment group （t = 2. 83,P 〈 0.01 ） , but the expenses were not added up markedly （t = 1.02,P 〉 0.05 ）. Conclusion The effect of non - operation comprehensive intervention to knee osteoarthritis in urban community is great which not only could exert the predominance of professional orthopaedics in general hospital but also felicitously show the excellence of health service center in urban community.
