摘要
目的观察短期胰岛素泵治疗对2型糖尿病患者胰岛β细胞功能及胰岛素抵抗的影响。方法用胰岛素泵对63例诊断时间为2-4年的2型糖尿病患者进行2周的持续皮下胰岛素输注(CSII),分析比较治疗前后空腹(FPG)及餐后2h血糖(2hPG)、糖化血红蛋白A1c(HbA1c),空腹胰岛素(FIns),空腹C-肽(FC-P),胰岛素分泌指数(Homa-β)、胰岛素抵抗指数(Homa-IR)。结果CSII治疗能快速稳定控制血糖,治疗2周后FINS,FC-P及Homa-β明显上升,Homa-IR明显下降(P〈0.01)。结论短期CSII治疗对诊断2-4年的T2DM有快速稳定控制血糖、改善胰岛β细胞功能和降低胰岛素抵抗的作用。
Objective To observe the effect of short - term intensive insulin pump on beta - cell function and insulin resistance in type 2 diabetic patients. Methods Sixty - three type 2 diabetic patients with a duration of illness of 2 - 4 years were treated by continuous subcutaneous insulin infusion (CSII) for two weeks. The levels of fast plasma glucose (FPG), 2 hours postpradial glucose (2hPG), and glycosylated hemoglobin Ale (HbAIe), fast insulin (Fins) and C - peptide (FC - P), insulin secretion indexes ( HOMA - B) and insulin resistance index ( HOMA - IR) were determined and compared before and after CSII. Results The level of blood sugar was well stably eontroled by CSII. The levels of Fins, FC - P, HOMA - B were increased significantly two weeks after intensive treatmetn with CSII. The HOMA- IR were decreased significantly. Conclusion Short- term CSII intensive treatment can stably control the level of blood sugar, improve the function of beta - cell and reduce the resistance to insulin in type 2 diabetic patients.
