抗精子抗体、抗子宫内膜抗体与习惯性流产关系的探讨

Relationship between antisperm antibody,anti-endometria antibody and habitual abortion

目的:探讨抗精子抗体(ASAB)、抗子宫内膜抗体(EMAB)与习惯性流产的关系。方法:用酶联免疫吸附试验(ELISA)检测受检者血清ASAB和EMAB,并分析其与习惯性流产的关系。结果:习惯性流产组的ASAB和EMAB阳性率分别为6.4%和33.0%,均明显高于对照组的3.4%和13.8%(P<0.01);早期流产(孕龄<12周)患者的ASAB和EMAB阳性率分别为9.8%和44.3%,明显高于晚期流产(孕龄≥12周)患者的0和12.1%(P<0.01);初次流产年龄<20岁患者两项免疫学指标(ASAB阳性或EMAB阳性)的总阳性率为64.9%,明显高于初次流产年龄≥20岁患者的35.1%(P<0.01)。结论:ASAB和EMAB与子宫内膜发生抗原抗体反应,干扰受精卵的着床和发育,导致反复流产,尤其是早期流产。

Objective： To make research on the relationship between antisperm antibody, anti-endometria antibody and habitual abortion. Methods： The patients＇serum antisperm antibody and anti-endometria antibody were determined by enzyme linked immunosorbent assay to analyze the relationship with habitual abortion. Results： The positive rates of antisperm antibody and anti-endometria antibody of habitual abortion group were 6. 38% and 33.0% respectively, apparently much higher than the comparison group which was 1.67% and 8. 33% （P〈0.01） respectively. The positive rates of antisperm antibody and anti-endometria antibody of the early abortion group （less than 12 weeks of pregnancy） were 83.3% and 0.9% respectively, remarkably much higher than the 16.7% and 29.1% of the late period habitual abortion group （12 or more weeks of pregnancy）（P〈0.01）. The total ositive rate of the two immunity quota of the first time abortion （less than 20 years patients） was 64. 9%, apparently much higher than the 35. 1% positive rate of the first ime abortion patients older than 20 years of age （P〈0.01 ）. Conclusion： Conclusion： Antlsperm antibody and anti-endometria antibody react with uterus internal membrane, disturbing the embedding and growth of the fertilized egg, creating anti-body reaction that causes repeated abortion, especially early abortion.
