摘要 目的:研究乳腺癌组织中基质金属蛋白酶-9(MMP-9)的表达情况及其与乳腺癌转移和预后的相关性,寻找能够有效判断乳腺癌转移潜能的分子生物学指标。方法:应用组织芯片技术和免疫组化方法(PV-9000法)检测106例乳腺浸润性导管癌石蜡包埋组织中MMP-9的表达情况,分析MMP-9的表达与淋巴结转移、肿块直径、病理分级、临床分期、患者年龄及雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体-2(HER-2)表达的关系,并分析MMP-9不同表达强度患者的生存率。结果:(1)106例乳腺癌组织中MMP-9表达阳性率为81.13%,随肿块直径、病理分级、临床分期的增加呈明显升高趋势,差异有统计学意义(P<0.05),腋淋巴结无转移者MMP-9阳性表达率((71.4%)明显低于腋淋巴结有转移者(87.5%)(P<0.05);(2)乳腺癌组织MMP-9表达与ER、PR、Her-2的表达之间不存在关联关系(P>0.05)。(3)MMP-9表达越强,患者术后生存率越低(P<0.05)。结论:MMP-9的表达在乳腺癌淋巴结转移中起重要作用,MMP-9可作为乳腺癌预后不良的独立参数。 Objective: To investigate the expressions of matrix metalloproteinase 9 protein (MMP-9) in breast carcinoma cells and analyze their correlations, trying to find the molecular biological markers for evaluating the metastatic potentiality of the breast carcinoma. Methods: The paraffin-embedded specimens from 106 breast invasive ductal carcinoma patients being made to tissue chips were stained for detection of expressions of MMP-9 with the immunohistochemical method, studying correlations of expressions of MMP-9 in the breast carcinoma with lymph node metastasis, tumor sizes, pathological grading, et al. Result: (1) The positive expression rate of MMP-9 in 106 breast invasive ductal carcinoma tissue was 81.13%. The expressions of MMP-9 were correlated positively with tumor sizes, pathological grading, clinical staging (P 〈 0.05, r =0. 274, 0. 327, 0. 260). The expressions of MMP-9 in the group without lymph node metastasis (71.4%) were lower than the expressions of MMP-9 in the group with lymph node metastasis (87.5%),the two groups had significant difference (P 〈0.05). The expressions of MMP-9 were no correlations with the expressions of ER, PR, HER-2 (P 〉0.05). (3) The higher the positive expressions of MMP-9 were, the patient survival rates were lower (P 〈0.05). Conclusion: The expression of MMP-9 plays an important role in the metastasis of lymph node in breast carcinoma. MMP-9 can be an independent prognostic indicator to breast carcinoma.
