幽门螺杆菌刺激慢性荨麻疹患者外周血单个核细胞产生白介素4和γ干扰素的意义

The effects of Helicobacter pylori on the production of IL-4,IFN-γ in the peripheral blood mononuclear cells from patiens with chronic urticaria

目的:探讨幽门螺杆菌(Hp)刺激慢性荨麻疹患者外周血单个核细胞(PBMC)产生白介素(IL)-4和干扰素(IFN)-γ及其意义。方法:采用ELISA法检测80例慢性荨麻疹患者和50名正常对照者的血清Hp抗体(Hp-IgG),随机选择25例慢性荨麻疹患者(Hp抗体阳性14例,Hp抗体阴性11例)、18名正常者(Hp抗体阳性9例,Hp抗体阴性9例),以Hp超声粉碎菌或植物血凝素(PHA)分别刺激其体外培养的PBMC,ELISA法检测PBMC培养上清中的IL-4和IFN-γ水平。结果:在Hp或PHA刺激下,Hp抗体阳性及Hp抗体阴性的慢性荨麻疹患者PBMC产生IL-4的水平均显著高于正常组(P<0.01);IFN-γ水平显著低于正常组(Hp抗体阳性患者)或无显著差异(Hp抗体阴性患者);但Hp抗体阳性与阴性患者PBMC产生的IL-4及IFN-γ水平相比,差异均无统计学意义。结论:入组的25例慢性荨麻疹患者PBMC经Hp刺激,可导致Th细胞亚群失衡,Th2免疫应答占优势。

Objective： To investigate Helicobacter pylori （Hp） stimulating the expression of IL--4 and IFN-γ in the peripheral blood mononuclear cells （PBMC） of the patients with chronic urticaria. Methods： Hp-specific IgG antibody was determined by enzyme-linked immunosorbent assay （ELISA） in 80 patients with chronic urticaria and 50 healthy controls. Then the levels of IFN-γ IL-4 in the supematants of PBMC stimulated with sonicated Hp or PHA were measured by ELISA in 25 patients with chronic urticaria （14 seropositive / 11 seronegative for Hp） and 18 healthy controls （9 seropositive / 9 seronegative for Hp）. Results： In chronic urticaria group （including the seropositive and seronegtive patients）, both HP and PHA induced the significant higher levels of IL-4 production than those in the healthy group （P〈 0.01）. While the levels of IFN-γ were significant lower in the chronic urticaria group with sero-positive to Hp than that in the whole healthy group, but no significant differences were detected between the chronic urticaria group with sero-negative to Hp and the whole healthy group. However, the levels of IL--4 and IFN-γ were no significant differences between the seropositive patients and the seronegtive patients. Conclusions： After stimulating of Hp in PBMC, the subgroups of T helpers in 25 patients with chronic urticaria might have a deviation, which would lead to priority of Th2 cytokine secretion.
