特发性脊柱侧凸椎间盘Ⅰ、Ⅱ型胶原表达的研究

摘要 目的:通过研究青少年特发性脊柱侧凸(AIS)患者椎间盘不同部位I、II型胶原表达的异常,总结胶原变化情况,探讨胶原代谢在脊柱侧凸发生发展中的作用。方法:取25例AIS患者经前路松解、骨骺阻滞或前路矫形手术切除之顶椎区的椎间盘,分纤维环的凹侧、凸侧两部分。其中取胸椎椎间盘(T8-11)11例,腰椎椎间盘(L1-2)14例。利用RT-PCR的方法逆转录合成I型胶原(220bp)和II型胶原(359bp),1%琼脂糖凝胶电泳,Gelwork图像分析系统进行半定量分析研究。结果:AIS患者顶椎区椎间盘纤维环I、II型胶原的表达凸侧较凹侧高(P<0.05)。腰椎表达较胸椎高,但除腰椎凹侧椎间盘纤维II型胶原的表达高于胸椎对应部位II型胶原的表达外(P<0.01),其余部位均无统计学差异。结论:AIS患者椎间盘纤维环的I、II型胶原的表达存在异常,且不同节段的表达存在差异。提示AIS患者的椎间盘发生变性,椎间盘胶原的代谢与AIS的发生密切相关,可能是导致AIS发生的原因之一,是脊柱侧凸进展的重要因素。 Objective:To study the distribution of the collagen Ⅰ and Ⅱ in intervertebral disc (IVD) of adolescent idiopathic scoliosis (AIS), summarize the changes of collagen, and investigate potential roles of collagen metabolism in the pathogenesis and development of AIS. Methods:25 cases of AIS were covered in this study. The IVD in apex curve were harvested during operation of slaking anterior part and epiphysis arrest or orthomorphia and then divided into two parts consisting of concave and convex annular fibrosus. Eleven specimens were intervertebral disc of thoracic vertebra ( from T8 to T1 1 ), other 14 were disc of lumbar vertebra ( from L1 to L2 ). The RT-PCR technic was used to synthesize collagen Ⅰ (220 bp) and collagen Ⅱ (359 bp). 1% agarose gel electrophoresis and Gelwork image analysis system were used in the semiquantitative analysis of the product. Resuits:The expressions of collagen Ⅰ and Ⅱ in convex annular fibrosus were higher than that in concave side (P 〈 0.01 ). The same change was happened in thoracic and lumbar annular fibrosus, but there were no statistic difference expect for the expressions of type Ⅱ collagen protein in the convex side of annular fibrosus in lumbar vertebra is higher than those in corresponding part of thoracic vertebra (P 〈0.05). Condusion:Both expressions of collagen Ⅰ and Ⅱ are abnormal in annular fibrosus from AIS patients and have significantly difference in various segments of vertebral column, with asymmetric expressive distribution between concave and convex sides. Our findings indicate degeneration in intervertebral disc of AIS, suggesting collagen metabolism have close relationship with development of AIS. Abnormal collagen metabolism maybe one of causes resulted in genesis of AIS and probably is an important factor in the progression of AIS.
