复杂髋臼骨折的治疗

Treatment Strategies of Complex Acetabular Fractures

【目的】探讨复杂髋臼骨折的治疗策略。【方法】总结本院1997-2004年经X线片、三维CT重建诊断的37例有移位复杂髋臼骨折治疗的经验。【结果】35例患者获得随访，平均随访时间为4．1（1．5～9）年。根据Matta解剖学评分，解剖复位20例，满意复位12例，不满意复位3例。根据D’Aubigne-Postel功能评定标准，优22例，良6例，可5例，差2例。【结论】完善的影像学资料，正确的术前诊断，适当的手术时期和手术方法，解剖复位，早期功能锻炼，降低并发症的发生可提高复杂髋臼骨折的治疗效果。

[Objective]To discuss the treatment strategies of complex acetabular fractures. [Methods]The experience in treatment of 37 cases of complex acetabular fractures treated operatively from 1997 to 2005 was summarized. They were diagnosed by X-ray ,CT and 3-DCT. [Results]Thirty five cases were followed up from 1.5 to 9 years with an average time of 4. 1 years. They were evaluated according to Matta anatomic criteria ,20 cases with anatomic reduction, 12 cases with satisfied reduction,3 cases with unsatisfied reduction, and evaluated by D＇Aubigne-pestel criteria for joint functions, 22 cases with excellent,6 cases with fine, 5 cases with fair ,2 cases with poor. [Conclusion]Good image data, correct preoperative diagnosis , appropriate operation time and approach,anatomical reduction , early functional rehabilitation, decrease of complication are essential for better treatmemt results of complex acetabular fractures.
