Stargardt病的临床和治疗观察

Clinical and therapeutical observation of Stargardt′s disease

目的观察Stargardt病的临床表现及其有意义的治疗。方法通过眼科常规检查及FFA、P-VEP和F-ERG等特殊检查分析本病特点,并给予中药治疗,观察治疗前后患者视力、P100振幅与潜伏期时值的改变。结果42眼中30眼视力0.1～0.3,眼底黄斑部有典型的金箔样反光,多有近视且视力不能矫正。FFA示萎缩区透见荧光,可有脉络膜湮没症;P100振幅明显降低,潜伏期轻度中度延迟;F-ERG多正常,闪烁光ERG振幅降低;治疗后视力提高,P100振幅改善。结论对Stargardt病的黄斑部典型金箔样反光、FFA、P-VEP和F-ERG检查是有价值的诊断,四物五子汤等中药有治疗意义。

OBJECTIVE To investigate the clinical characters and significative treatment of Stargardt＇s disease.METHODS Twenty - one patients（42 eyes）with Stargardt＇s disease were tested with general check - up of ophthalmology. FFA and F- ERG performed in partial patients, and treated with Siwu Wuzi decoction（ 四物五子汤）for 1 to 3 months. RESULTS The visual acuities of thirty eyes were 0.1 to 0.3. The typical copper color was found in the macular lesion of ocular fundus. On fluorescein angiography, window defect - hyperfluorescence was revealed. Choroidal silence sign was found in 53 % cases. Abnormity was revealed by P- VEP and Flicker- ERG. The visual acuity and P- VEP were improved. CONCLUSIONS Copper color in the macular lesion of ocular fundus, P - VEP, FFA and Flicker - ERG had diagnostic values. Siwu Wuzi decoction was effective to treat Stargardt＇s disease.
