微创手术治疗锁骨骨折198例临床分析

Clinical analysis of treatment of clavicle fracture with the minimally invasive in 198 cases

目的评价微创治疗锁骨骨折的临床疗效及讨论治疗体会。方法对198例锁骨骨折,中1/3段骨折95例,中、外1/3段交界处骨折69例,均采用微创手术治疗,术后三角巾悬吊保护4-6周。结果经术后5-18个月、平均10个月随访,术后骨折愈合时间3-6个月,平均3.5个月。5例出现克氏针折弯,1例出现骨折骨不连,再次手术。按照韩平良等锁骨疗效标准,优103例;良89例;可5例;差1例。优良率96.5%。结论微创治疗锁骨中1/3段骨折以及中、外1/3段交界处骨折,并且不存在肩锁关节脱位的损伤,符合生物力学固定原理和微创治疗原则,操作简单,固定稳妥,功能恢复满意。

Objective To evaluate the clinical effects of clavicle fracture treated with minimally invasive treatment. Methods Among 198 cases of clavicle fracture, 95 cases were mid-clavicle fracture, 69 cases were mid-outside clavicle fracture, all were treated with minimally invasive treatment, the upper limbs of the operated side overhang with gore about 4 to 6 weeks post operation. Results The post-operation follow-ups ranged from 5 to 18 months with an average of 10 months, the average clinical healing period was 3.5 months （ from 3 to 6 months）. 5 cases suffered blent of Kirschner wire, and 1 case suffered delayed union. According to the effect standard of Han Ping-liang, excellent 103 cases, good in 5, fair in 3, poor in 1, the excellent and good rate was 96.5%. Conclusion Minimally invasive treatment is an effective method to mid-clavicle fracture and the joint of mid-outside clavicle fracture because of its consistent with vitodynamics and minireally invasive principle, convenient for clinical application, reliable fixation, satisfied with functional recovery.
