外部性脑积水患儿运动发育迟缓早期干预治疗的GMFM评价

摘要 目的 研究小儿脑瘫运动评估量表(GMFM)对外部性脑积水(EH)患儿运动发育迟缓粗大运动功能及早期干预的疗效评价。方法 30例头颅CT诊断为EH的运动发育迟缓儿,4-6个月19例,9个月11例。采取中西医神经康复和早期干预治疗方案干预治疗3个月及使用神经营养剂。治疗前后粗大运动功能采用GMFM评价。结果 4-6个月组治疗前后在卧位和翻身、坐位结合平衡反射方面具有显著疗效。9个月组治疗前后在卧位和翻身、坐结合平衡反射、四点位及跪爬方面具有显著疗效。两组总评分在治疗前后差异亦有显著性。结论 GMFM不仅能反映EH运动发育迟缓的客观指标,而且能够评估早期干预治疗运动功能的改善。 Objective To evaluate early interfering treatment to hypoevolutism in motor development pediatric patients with external hydiocephalus (EH) . Methods There were 30 EH pediatric patients diagnosed by CT scan , among them , 9 were 4 - 6 months old , 11 were - 9 mouth old . The integrated Chinese - western medical treatment and early stage interfering treatment together with neurotrophy were applied to treat for 3 months . The gross motor function measure(GMFM) was used to evaluate the gross motor function before and after the treatment . Resutis Pediatric patients in group of 4 - 6 months old improved significantly in balance reflection between recumbent position and rolling , sitting positions . While in - 9M group , they improved significandy in balance reflection between connection of recumbent position and rolling . sitting positions, four o' clock position and creeping with knees . There was significant difference between the two group in scoring . Conclusions GMFM is not only an index to reflect the hypoevolutism in motor development , but could be used to evaluate the improvement of motor function after early interfering treatment.
