摘要
目的:采用碱性成纤维细胞生长因子干粉剂并中药治疗放射性溃疡,探索一种新的保守疗法对放射性溃疡的治疗效果。方法:选择2000-03/2006-06北京积水潭医院烧伤科收治放射性溃疡患者18例共18个创面。男5例,女13例;年龄35~75岁;病程6个月~4年。患者均知情同意。所有溃疡创面清创后采用碱性成纤维细胞生长因子干粉剂(北京双鹭药业有限公司生产)与橡皮生肌膏(天津达仁堂生产)混合内敷,外用溃疡贴(丹麦康乐保公司生产)的方法,每两三天换药1次。观察溃疡创面愈合状况及完全愈合所需时间。由于入选的病例均是经过常规方法治疗半年后尚未愈合的创面,故在实验中除统计每一个创面完全愈合所需的时间外,还以6个疗程(42d)作为疗效判定标准。结果:18例患者均进入结果分析。18例溃疡创面都产生了明显的愈合,其中3周内愈合者1例,4周内愈合者7例,5周内愈合者4例,6周内愈合者3例,超过6周愈合者3例(其中2例植皮后愈合),6周内总愈合率为83.3%,愈合所需时间平均为34.4d。追踪随访14例患者,至少半年内没有复发,最长的已达3年,另有4例出院后失去联系,未能随访。结论:采用碱性成纤维细胞生长因子干粉剂与橡皮生肌膏混合内敷,外用溃疡贴保守疗法能够有效地促进放射性溃疡的愈合。
AIM: To study the therapeutic efficacy of a novel conservative treatment to roentgen ulcer by the combination of basic fibroblast growth factor (bFGF) powders and Chinese herbs.
METHODS: A total of 18 patients with roentgen ulcer were admitted in Department of Bums, Beijing Jishuitan Hospital from March 2000 to June 2006, including 5 males and 13 females, aging 35-75 years, and their course of disease ranged from half a year to 4 years. All the subjects were informed of the scheme detection and agreed before participation. An admixture, which consisted of bFGF powders (Beijing SL Pharmaceutical Co., Ltd.) and granulation-enhancing mastics (Tianjin Darentang Pharmaceutical Factory), was applied on the ulcer wound surface. In addition, one kind of ulcer sticking (Denmark Coloplast Group) was used externally. Above mentioned dressings were changed every two or three days. The condition of ulcer wound and healing time were constantly observed. All the adopted cases were unhealed after routine treatment of half a year, thus the time of complete healing in each case was determined, and curative effect was assessed in 6 courses of treatment (42 days).
RESULTS: All 18 cases entered the result analysis and were healed in wound, 1 case within 3 weeks, 7 cases in 4 weeks, 4 cases in 5 weeks, 3 cases in 6 weeks, 3 cases exceeding 6 weeks; total healing rate within 6 weeks was 83.3%, and average healing time was 34.4 days. Moreover, 14 patients in follow-up appeared no relapse within half a year to three years, while other 4 patients were not kept in touch.
CONCLUSION: This novel conservative treatment is effective in accelerating the healing of roentgen ulcer, comprising a mixture of bFGF powders with granulation-enhancing mastics and an extemal application of ulcer sticking.
