福建省实施DOTS策略防治结核病的成本效益分析

Cost Benefit Analysis of Tuberculosis Control Project by Carrying Out DOTS Strategy in Fujian,China

[目的]研究结核病防治资金投入与社会经济效益的关系,评价DOTS策略在我省实施的效果。[方法]对2001—2005年全省结核病防治中投入的经费与病人发现和治疗情况进行分析。[结果]5年间共投入经费6255万元,发现活动性肺结核病人112893例,其中涂阳病人614253例,治愈53291例;使53.3～79.9万人免受感染,减少新发病人2.7～4.0万;治愈病人并减少新发病人的发生,共挽回生命质量调整年841970～983193年,相当于挽回11 362～3268个健康生命;DOTS方案与传统方法比较治疗费用节省3658万元;减少传染而节省的费用3122～4695万元;减少病人误工损失4743万元。[结论]采用DOTS防治结核病经济效益和社会效益显著,符合疾病控制与卫生经济学成本-效益原则。

[Objective] To study the relationship between financing input and social and economic benefit of TB control project by carrying out DOTS strategy in Fujian. [Methods] Financing input, TB cases detecting and treatment were analyzed for the project from 2001 to 2005. [Results] Totally 62. 5515 million Yuan （RMB） was inputted; 112 893 active TB cases were detected（614 253 smear positive cases were detected and 53 291 smear positive cases were cured）. The project benefited to 533-799 thousand people, avoiding TB infection. 26.6-40 thousand new TB cases reduced. Totally 841 970-983 193 DALY years were retrieved, corresponding with 11 362-13 268 healthy people. Totally 36. 5751 million Yuan was saved by DOTS strategy, comparing with tradition ways. And 31. 2223-46. 9509 million Yuan was saved because of reducing infection. And 4. 74343 million Yuan was saved because of missing work hour. [Conclusion] The economic and social benefit is significant with DOTS strategy of TB control and the strategy is according with costbenefit principle.
