54例胰体尾癌根治术后生存率分析

Radical resection of neoplasms of the body and tail of the pancreas:survival analysis of 54 cases

目的:分析影响胰腺体尾癌术后生存因素。方法:随访我科2002年至2006年间54例根治性胰体尾癌切除术病人,用寿命表法进行生存率分析,并研究与预后相关的危险因素。结果:69例病人的1年生存率为68.8%,2年生存率为56.1%,3年生存率为34.2%,中位生存期23.6月。性别、年龄与术后生存率无关(P>0.05)。有局部淋巴结转移、神经侵犯病人的术后1年生存率显著低于无局部淋巴结转移、无神经侵犯者(P值均<0.05)。胰体尾联合脾切除术的不同残端处理方式间的生存率差异无统计学意义(P>0.05)。结论:胰体尾癌局部淋巴结转移和神经侵犯情况与其预后之间有密切联系。

Objective To investigate the factors influencing the outcome of patients with neoplasm of the body and tail of the pancreas submitted to radical resection. Methods The clinical data of 54 patients were analyzed retrospectively, Kaplan-Meier （Log-rank） method was used to compare the survival rates. Results Among the 54 cases, the 1, 2 and 3 year survival rates were 68.8%, 56.1%, and 34.2%, respectively. The median survival time was 23.6 months. Age, gender, tumor size, and the management of the pancreatic stumps, were not suitable to serve as prognostic factors. The 1 year survival rate in patients with regional lymph nodes metastasis and nerve invasion was significantly lower than those without LN metastasis and nerve invasion. Conclusions Status of regional lymph node metastasis and nerve invasion might be employed as prognostic factors in patients with neoplasms of the body and tail of the pancreas.
