过伸体位下经皮椎体成形术治疗椎体骨质疏松性骨折 被引量:11

Treatment of osteoporotic vertebral fracture by percutaneous vertebroplasty in the extend position

摘要 目的研究过伸体位下经皮椎体成形术治疗骨质疏松性椎体骨折的临床疗效。方法过伸体位下经皮椎体成形术治疗骨质疏松性椎体压缩性骨折35例,38个椎体,均为新鲜骨折,无神经损伤症状。术前显示椎体后壁均完整。先在过伸体位下使病椎的高度恢复,然后在C臂透视下,经椎弓根穿刺,向椎体内注入含钡的骨水泥。结果所有患者术后疼痛均迅速缓解,术前VAS评分为8.53±1.23,术后为2.43±0.82。椎体被压缩的高度有明显恢复,术前为17.3±3.5mm,术后为21.6±2.3mm。所有患者均未出现肺动脉栓塞等并发症。结论过伸体位下经皮椎体成形术治疗骨质疏松性椎体压缩性骨折可以有效缓解疼痛,部分恢复椎体高度,临床疗效满意。 Objective To investigate the clinical outcomes of percutaneous vertebroplasty in the extend position to manage osteoporotic vertebral compression fractures. Methods Percutaneous vertebroplasty in extend position was performed in 38 vertebrae of 35 patients, which were all fresh fractures, without neurotic symptoms and signs. The posterior vertebral walls were all intact in all patients under CT scan. In the extend position, the height of the compressed vertebral body would be restored. Then under the guidance of C-type arm X ray unit, the vertebral body was filled with bone cement through pedicle of vertebral arch by percutaneous puncture. Results The pain was alleviated after the operation. The preoperative scores of VAS were 8.53 ± 1.23 and the postoperative scores were 2.43 ±0.82. The heights of the compressed vertebral body were restored evidently. There were no complications in this group of patients. Conclusions Percutaneous vertebroplasty in extend position can relieve the pain and restore the compressed vertebral height evidently with better outcomes.
