应用体内标记^(99m)Tc-RBC和SPECT诊断肝海绵状血管瘤

摘要 目的评价体内标记99mTc-RBC和SPECT对肝海绵状血管瘤的诊断准确性。方法对219例肝血池显像进行分析。结果根据瘤体直径本组显像阳性率分别为:直径小于1.5cm者为40%,1.5~2.0cm者为72.1%,2.0~4.0cm者为97.3%,4.0~6.0cm者为100%,大于6.0cm者为100%。结论肝血池显像诊断肝脏海绵状血管瘤,尤其是直径大于2.0cm的海绵状血管瘤具有良好的特异性,是一种无创、安全、便捷的检查方法。 Objective To assess the diagnostic accuracy of 99m^Tc -labelled red blood cell(RBC)and single -photon emission computed tomography(SPECT)for hepatic cavernous hemangiomas. Methods Two hundred and nineteen patients with hepatic cavernous hemangiomas which were demonstrated by US, X - ray, CT and follow - up observation underwent hepatic blood - pool imaging with 99m^Tc - labelled RBC scintigraphy and SPECT. The imaging findings in all patients were retrospectively analyzed. Results The detective rate of hepatic cavernous hemangiomas with different diameters was varied i. e the aetective rate was 40% for tumoral lesions smaller than 1.5 cm in diameter, 72. 1% for 1.5 -2.0 cm , 97.3% for 2.0 -4.0 cm, and 100% for 4.0 -6.0 cm or lager, respectively. Conclusion 99m^Tc - labelled RBC scintigraphy and SPECT have high specificity in the diagnosis of hepatic cavernous hemangioma, especially in those larger than 2.0 cm in diameter and therefore it is a non - invasive, safe and convenient means.
