超声检查网膜囊病变在急性胰腺炎诊断中的临床意义

Clinical significance about ultrasonography of omental sac lesions for diagnosing the acute pancreatitis

目的提高对网膜囊病变超声表现的认识并评价其在急性胰腺炎(AP)诊断中的临床意义。方法回顾性地分析了104例AP病例,其中,伴发网膜囊病变者38例,对网膜囊病变的类型,声像图特征,及其与胰腺超声检查结果的关系进行了研究。结果本组网膜囊病变表现为网膜囊增厚型和网膜囊积液或积脓型2种类型。本组网膜囊病变总的发生率为36.5%(38/104),网膜囊增厚型为14.4%(23/104),网膜囊积液/积脓型为22.1%(23/104)。结论网膜囊病变是AP的一个重要的间接征象,对AP的诊断有重要提示作用,一旦发现网膜囊病变则对AP特别是声像图改变不明显的AP的诊断有重要的临床意义。

Objective To elevate the cognition of omental sac lesions on ultrasonographic images and to assess its significance for diagnosing the acute panereatitis（AP）. Methods A hundred and four patients with AP in which 38 cases complicated with omental sac lesions were analyzed retrospectively. The pathologic types and ultrasonographie features of omental sac lesions and the correlation to ultrasonographie findings of panereas were carefully investigated. Results The lesions of omental sac in 38 patients presented both types of omental sac thickening and hydrops（or empyema）. The incidence of omental sac lesions in the group was 36. 5% which comprised 14.4% of omental sac thiekning and 22. 1% of omental sac hydrops（or empyema）. Conclusion Omental sac lestions are important indirect signs of AP and have the suggestive role for diagnosing AP. Once omental sac lesions occur, especially when ultrasonographie findings of AP are atypical, those lesions have important significance for diagnosing AP.
