Oddi’s括约肌切开成形术43例疗效分析

Clinical analysis on the therapeutic effect of Oddi's spincterplasty in 43 cases

目的探讨Oddi’s括约肌切开成形术的疗效。方法回顾分析43例括约肌切开成形术的临床资料。其中胆囊切除术后括约肌狭窄11例,胆管壶腹部结石嵌顿3例,胆总管切开取石伴胆管末端狭窄19例,复发性胆源性胰腺炎6例,内镜下括约肌切开术后4例;第1次手术24例,第2次手术19例。结果腹部切口感染5例,短暂性胆漏2例,括约肌手术处渗血3例,37例(占86%)经5￣15年随访,效果满意。结论正确掌握手术适应证和操作方法,可取得良好效果。

Objective To evaluate the curative effect of Oddi＇s spincterplasty. Methods Clinical date of Oddi＇s sphincterplasty were retrospectively sunmerized in 43 cases during recent 20 years. Of whom,male was 20, female 23, aged 43-65, narrow of sphincter post-cholecystectomy in 11 cases, stone impact in the Vater＇s ampullar in 3 cases, post-choledocholithotomy with stenosis of choledochal end in 19, recurrent biliary pancreatitis in 6, post- EST in 4. First biliary operation in 24 cases and the second biliary surgery in 19. Results Incisional infection occurred in 5 cases, transient bile-leak in 2,and errhysis in 3. 37 cases （86%） of 43 Oddi＇s spintcerplasty were followed up for 5-15 years, and the effect was excellent. Conclusions Correctly grasping the indication and method of this operation may gain good results.
