糖尿病足感染常见病原菌及抗生素选择的相关问题 被引量:6

摘要 目的根据糖尿病足(DF)创面分泌物标本的培养结果和药敏试验,探讨糖尿病足感染的常见致病菌以及合理选择抗生素的相关问题。方法收集自2003年10月到2006年10月因糖尿病足并发感染入住我院骨科、外科和内分泌科的128例病人,由专人采集患足创面的分泌物标本做细菌培养和药敏试验,并对结果进行分析。结果128例标本共培养出病原菌137株,其中细菌135株(98.5%),真菌2株(1-5%),多株混合菌感染共17例(13-3%)。前5位的致病菌为金黄色葡萄球菌(19株)、粪肠球菌(17株)、大肠埃希氏菌(15株)、溶血葡萄球菌(14株)、铜绿假单胞菌(11株)。结论DF感染常为细菌性感染和多病原菌的混合感染,临床上应选用杀菌性广谱抗生素为主的联合用药。 Objective To discuss the common pathogen and proper choice of antibiotics for diabetic foot infections on the basis of culture results and susceptibility test of secretion taken from the raw surface. Method 128 patients hospitalized for diabetic foot infection in the department of endocrine, surgery and orthopedics from October 2003 to October 2006 were enrolled and analyzed. Secretion sample were taken from the raw surface and the bacterial culture and susceptibility test were performed in lab. Results 128 samples were cultured and 137 strains of pathogen were found, 135 strains were bacterium (98.5%), 2 strains were fungi (1.5%), 17 were multi-pathogen infection (13.3%). The top 5 pathogenic bacteria were: SA (19 strains), Enterococcus faecalis (17 strains), Escherichia coli (15 strains), SH (14 strains), Pseudomonas aeruginosa (11 strains). Conclusion Bacterial infection and multi-pathogen infection are common in diabetic foot infections. Bactericidal broad-spectrum antibiotics should be chosen in combination.
