巨块型宫颈癌术前介入化疗与放疗的疗效对比分析

Effects of Intra-arterial Neoadjucant Chemotherapy and Radiotherapy Before Radical Hysterectomy of Massive Cervical Cancer

目的比较术前介入化疗与传统放疗治疗Ⅰb～Ⅱa期巨块型宫颈癌的疗效,以探讨Ⅰb～Ⅱa期巨块型宫颈癌术前介入治疗的临床疗效。方法对82例Ⅰb～Ⅱa期巨块型宫颈癌患者分组进行盆腔介入化疗与放疗,经治疗后3周进行宫颈癌根治术,评定两组的疗效。结果介入组比放疗组的宫颈肿瘤消退明显,总有效率(CR+PR)%分别为88.1%和70.0%,差异有显著性(P<0.05);介入组与放疗组的淋巴结转移率分别为16.6%和17.5%,差异无显著性(P>0.05);两组的1～3年生存率差异无显著性。结论术前介入化疗治疗可以作为宫颈癌综合治疗的一种有效方法。对Ⅰb～Ⅱa期巨块型宫颈癌有满意的近期疗效。

Objective To study the therapeutic effects of preoperative intra- arterial chemotherapy and radiotherapy before radical excision in patients with massive cervical cancer in stage Ib-Ila. Methods Eighty- two patients with massive cervical cancer in stage Ib- Ila were assigned into two groups and received operative intra - arterial chemotherapy and radiotherapy respectively before radical excision. Results The patients received preoperative intra- arterial chemotherapy achieved better results than the patients received preoperative radiotherapy （P〈0.05）. The rate of pelvic lymphnode metastasis between two groups showed no significant difference （P〉0.05）. Conclusien The preoperative intra- arterial chemotherapy preparation has better therapeutic effect than those of the traditional preoperative radiotherapy in treatment of massive cervical cancer in stage Ib- IIa.
