脊膜膨出患者人工反射弧手术前的神经电生理评估

摘要 目的探讨神经传导检查对脊膜膨出合并尿失禁患者人工反射弧手术前评估的应用价值。方法总结自2001年以来94例脊膜膨出拟接受人工反射弧手术的患者术前胫神经、腓浅神经和腓总神经传导检查的结果,结合部分患者术后尿动力学检查了解手术效果。结果94例患者中,双侧传导速度和波幅均正常[(54.5+7.4)m/s,(9.3+3.4)mV]的75例;单侧胫神经传导速度[(35.1+5.6)m/s]降低有10例,波幅降低(4.5+2.1)mV7例,单侧腓总神经传导速度[(28.4+11.8)m/s]降低11例,波幅降低(4.7+2.9)mV8例。11例单侧神经传导速度降低患者选择在传导功能更好的一侧手术;8例病人因双侧神经传导速度减退,并且波幅明显下降,而未接受手术。结论术前神经传导功能检查对了解新反射弧传入神经腰5脊神经前根传导功能及选择吻合的神经具有重要的价值。
