摘要 目的探讨全身MR扩散加权成像(MR-DWI)在3.0T磁共振上的正常表现。方法42例健康志愿者及10例恶性肿瘤患者行3.0T磁共振全身MR-DWI检查,初步归纳全身MR-DWI的正常表现,比较不同性别骨骼系统信噪比(SNR)值的差别,并对骨骼系统进行年龄与SNR值的相关性分析。结果健康志愿者骨骼呈低至等信号,双肺、纵膈及肝脏呈低信号,椎间盘、颅脑、脾脏、双肾和子宫肌层/前列腺外周带呈高信号。女性腰椎、髂骨及股骨SNR值显著高于男性,且女性腰椎、髂骨SNR值与年龄呈负相关。10例恶性肿瘤患者全身MR-DWI发现原发灶以外的59处异常信号病灶。结论骨骼系统MR-DWI的SNR值受到性别和年龄的影响;全身MR-DWI对显示肿瘤转移灶非常敏感,有可能成为探测肿瘤病人全身转移灶的一种手段应用于临床。 Objective To investigate the normal appearances of whole body diffusion weighted imaging (DWI) at 3.0T MR scanner. Methods Forty-two healthy volunteers and 10 patients with malignant tumors underwent whole body DWI examination at 3.0T MR scanner, and the normal appearances of whole body DWI were assessed. The difference of signal-to-noise ratio (SNR) values in the skeletal system between genders and the correlation between age and SNR values in skeletal system were analyzed. Results In healthy volunteers, the skeletal system showed low to isointensity, the lungs, mediastinum, and liver showed low signal intensity, while intervertebral discs, brain, spleen, kidneys, muscular layer of uterus, and peripheral zone of prostate demonstrated high signal intensity. Besides, SNR values of lumbar vertebrae, iliums, and femurs in females were significantly higher than those in males, and there was significant negative correlation between age and SNR values of lumbar vertebrae and iliums in females. Besides, whole body DWI detected 59 metastatic lesions in 10 patients with malignancy. Conclusion The SNR value of the skeletal system was influenced by gender and age in whole body DWI. Since whole body DWI had a high detection rate for tumor metastases, it could be a useful method to detect the metastasis of whole body in patients with malignant tumors.
