PS-SOD脂质体对乳猪体外循环肺保护作用的研究

The protective effect of PS-SOD liposome on lung injury induced by extracorporeal circulation in piglets

摘要 目的探讨肺表面活性物质-超氧化物岐化酶(PS-SOD)脂质体在乳猪体外循环中的肺保护作用。方法选用2~3周龄乳猪30头,随机分为对照组、PS 组、SOD 组、PS+SOD 组、PS-SOD 脂质体组5组。常规建体外循环,转流150min,主动脉阻断60min。分别在气管插管后、主动脉开放前2个时点经气管插管向肺内均匀注入生理盐水、PS、SOD、PS+SOD、PS-SOD 脂质体。于术前、术毕、术后2、6和12h 5个时点检测乳猪肺顺应性(Cdyn)、氧合指数。测定术后肺组织中丙二醛(MDA)、髓过氧化物酶(MPO)、谷胱甘肽过氧化物酶(GSH-PX)的含量,并行肺组织形态学观察。结果 PS-SOD 脂质体组术后 Cdyn 与 PS 组及 PS+SOD 组比较无明显改变(P>0.05),与 SOD 组比较明显改善(P<0.05)。与术前比较,PS 组 SOD 组和 PS+SOD 组氧合指数均明显下降且回升不明显,而 PS-SOD 组下降不明显且术后迅速回升。PS+SOD 组与 SOD组比较,MPO、MDA 无明显下降,GSH-PX 无明显升高(P>0.05)。PS-SOD 脂质体组与 SOD、PS+SOD 组比较,MPO、MDA 明显下降,GSH-PX 明显升高(P<0.01)。PS+SOD 组肺病理形态学定量评估与 PS 组及 SOD组比较无明显减轻(P>0.05)。PS-SOD 脂质体组肺病理形态学定量评估与 PS、SOD、PS+SOD 相比较则明显减轻(P<0.05)。结论单纯应用 PS 或 SOD 均有一定的肺保护作用,但二者混合应用并未提高作用效果,PS-SOD 脂质体表现出更良好的肺保护效应。 Objetive To investigate the protective effect of sumfactant-super r oxide dismtses (PS-SOD) liposome on lung injury induced by extracoryored eirclation in piglets.Methods Thirty 2 - 3-week-old pigtets were randomly divided into 5 groups: contral group; PS group; SOD group; PS+SOD group; PS-SOD liposome group.The duration of extracorporeal circulation and aortic cross-clamp was 150 rains and 60 rains,respectively. Normal saline, PS, SOD, PS+ SOD and PS-SOD liposome were infusedd into the trachea cannnla at the points of the tracheal intubation and removing the clamp. Dynamic lung compliance (Cdyn) and oxygenation index (OI) were detected at the beginning, end of cardiopulmnary bypass(CPB) and 2h, 611, 12h after CPB. The concentration of malmdialdehyde(MDA), myeloperonidase(MPO), gtutathion peroxidase(GSH-PX) were also determined postopratively. We also ohserred the pathdogical chages the lung after CPB. Results Compared with PS group and PS + SOD group , the Cdyn of PS-SOD lipoe,The group did not change abviously after the operation ( P 〉 0.05). The Cdyn of PS-SOD liposome group was higher than that of SOD group ( P 〈 0.05). The OI of PS group, SOD group, PS + SOD gnmp decreased distinctly and rose slowly after the operation, but the O1 d PS-SOD lipoeme group did not decrease so much and had a quick raise; The concentration of MDA, MPO and GSH-PX was basically the same between SOD group and PS+ SOD group (P 〉 0.05). The concentration MMPO, MI)A in PS.-SOD lipoeme group was lower than that of SOD group and PS + SOD group ( P 〈 0.01 ), and the concentration of GSH-PX was higher. The lung in PS-SOD lipoeme group was injured more slighdy. Conclusion While both PS and SOD perform protective effect on lung injury induced by CPB, combination of these two not produces additive eontribution. Administration of PS-SOD lipsome may play important role in lung preveation.
