骨科诊疗规范自动监控系统的实现

Accomplishment of an Automatic Monitoring System for Orthopedics Rules of Diagnosis and Treatment

在疾病的治疗过程中,手术适应症扩大化、治疗方法选择不适当等现象经常存在,不但增加了病人的痛苦和经济负担,同时也增加了医疗风险。规范医生临床诊疗行为的诊疗规范、临床路径等研究已取得了较大的进展,但规范的实际执行往往依赖于临床医生的自主性,缺乏刚性的流程约束和有效的监控措施,管理力度不够。为了实现医生临床诊疗行为的自动跟踪与监控,我们基于医院信息系统已有的数据库和网络架构,成功开发骨科诊疗规范自动监控系统。系统已经在我院膝部损伤科、足踝外科、髋部损伤科、小儿正骨科等十几个临床科室应用,运行良好,安全稳定,对提高医疗管理质量和效率起到了明显效果,提升了医院管理水平。

In the course of treatment of bone and soft tissue injuries, irregularities often come into being such as widening scope of indications for surgical operation ,improper selection of curative methods and so on. Physical suffering and economic burden are increased on patients and meanwhile,medical risks are enhanced. Research on medical rules to discioline doctors＇clinical behavior and approaches has made mrch progree, but to carry out the text rules usually depend on doctor＇s self - determination lacking strict flow control and effective supervision. In order to realize automatic tracking and monitor on doctor＇s clinical medical behavior,we have successfully developed the automatic monitoring system on the basis of the existing data bank and network of the hospital information system. The automatic monitoring system has been put into effect in over ten departments in our hospitsl such as departments of knee injuries, hip injuries, foot ang ankle such as departments of knee injuries, hip injuries , foot and ankle surgery, pediatric orthopedics and so on. It works well, safe and stable, giving remarkable impact on quality and efficiency of medical management, thus upgrading the administrative level of the hospital.
