缺氧缺血性脑病新生儿脑干听觉诱发电位及血清神经元特异性烯醇化酶变化及其临床意义 被引量:3

摘要 目的了解脑干听觉诱发电位(BAEP)和血清神经元特异性烯醇化酶(NSE)对评价缺氧缺血性脑病(HIE)新生儿预后的价值。方法选择窒息新生儿46例为HIE组,同期健康新生儿21例为健康对照组。常规留取窒息新生儿生后12~24h和HIE恢复期血清标本,同时留取健康对照组血清,在同等条件下分别进行BAEP、NSE检测。结果HIE组BAEPⅠ、Ⅲ、V波绝对潜伏期(PL)、峰间潜伏期(IPL)均明显延长,与健康对照组比较,差异有统计学意义(Pa<0.01);HIE组血清NSE明显高于健康对照组,差异有统计学意义(P<0.05)。结论BAEP和NSE可作为HIE患儿病情判断和预后评估的重要监测指标。 Objective To evaluate the role of brain stem auditory evoked potential (BAEP) and neuron specific enolase (NSE) in the diagnosis and prognosis of hypoxic - ischemic eneephalopathy(HIE) in newborn infants. Methods The HIE and normal term neonates were included. The HIE group contained 46 cases and control group contained 21 cases. Serum samples were studied in HIE term - newborn infants. Some in postnatal 12 -24 h and the others within postnatal 7 - 10 d as the symptoms and signs of HIE were developed while only serum samples were obtained from neonates in healthy term as controls. BAEP and NSE serum level were determined damage. Results Neonates with HIE had significantly prolonged latent period(PL) and internal latent period(IPL) of wave Ⅰ, Ⅲ ,Ⅴ in BAEP( Pa 〈 0.01 ), and raised wave V reaction threshold than those in control group(Pa 〈0.01 ). The serum level of NSE in neonates with HIE was significantly higher than that of control group ( P 〈 0.05 ). Conclusion BAEP and NSE can work as sensitive monitoring indexes to determine clinical severity and prognosis in HIE neonates.
