64层螺旋CT在不稳定型心绞痛危险分层中的作用

摘要 目的探讨64层螺旋CT在不稳定型心绞痛危险分层中的作用。方法收集2006年3月至2006年9月间36例不稳定心绞痛(UA)的冠心病患者按Braunwald class分三级,并行64层螺旋CT冠状动脉扫描检查,检测斑块分为三类:富含脂类的软斑块、纤维斑块、钙化斑块;病变支数分单支、双支和三支。然后对比UA各危险分层中不同斑块的性质和病变支数,以评价64层螺旋CT对不稳定心绞痛危险分层的意义。结果36例患者中Ⅰ级的患者15例,其中67%为单支病变患者(10例);Ⅱ级的患者14例,单支和多支病变者各占50%;Ⅲ级的患者7例,三支病变者5例占71%。随危险分级增高病变支数有增多趋势,且差异有统计学意义(P〈0.05)。在Ⅰ级的患者中软斑块最多,随危险分级增高钙化斑块及混合斑块也增多;斑块类型在Ⅰ级和Ⅲ级之间对比差异有统计学意义(P〈0.05)。结论64 MSCT冠脉造影扫描可以作为冠状动脉狭窄性冠心病的初步筛选手段,在不稳定型心绞痛危险分层中有重要意义。
