超声引导经皮经肝穿刺胆管置管引流术(附159例报告)

Percutaneous transhepatic-cholangial drainage guided by ultrasound:report of 159 cases

目的探讨超声引导下经皮经肝穿刺胆管置管引流术(PTCD)的临床应用价值。方法159例梗阻性黄疸病人,其中肝癌84例,胰头癌31例,胆管癌19例,胃癌肝转移12例,胆囊癌13例,在超声引导下经皮经肝胆管穿刺置管引流术178次。结果肝总管内置管86支,右肝管内置管41支,左肝管内置管51支,导管置入失败3支,PTCD成功率98%。结论超声引导下PTCD定位准确,成功率高,是较为安全、有效、病人易接受的胆道减压引流方法。

Objective To evaluate the clinical application of percutaneous transhepatic cholangial drainage （PTCD） guided by ultrasound in patients with obstructive jaundice. Methods PTCD were performed under the guidance of ultrasound 178 times in 159 patients with obstructive jaundice. The patients were 84 hepatic carcinomas, 31 pancreatic carcinomas, 19 cholangiocarcinomas, 12 hepatic metastasis from stomach, and 13 gallbladder carcinomas. Results The drainage catheters were placed successfully in common hepatic duct in 86 cases, in right hepatic duct in 41 cases, in left hepatic duct in 51 cases and 3 cases were failed. The successful rate was 98%. Conclusion PTCD under the guidance of ultrasound is a safe, effective and acceptable technique managed in patients with obstructive jaundice.
