摘要 目的探讨3D CE-MRA对下肢动脉硬化性闭塞症临床应用价值。方法检查40例疑有下肢动脉疾的患者行三维增强下肢磁共振血管成像(3D CE-MRA)检查。仪器为1.5T磁共振成像仪(Magneton Avanto,Siemens)配置32个接收通道和并行采集技术。其中24例患者接受彩色多普勒超声诊断仪检查。结果40例病例包括临床诊断或怀疑行CE-MRA中37例下肢动脉可见不同程度的狭窄及闭塞,阳性诊断率92.5%(37/40)。与手术结果的符合率为92.7%。而彩色多普勒超声诊断结果符合率为88%。结论3D CE-MRA对下肢动脉病变的显示具有独特的优势,是一种无创、安全、可靠、有应用价值的检查方法,甚至是首选的方法。 Objective To discuss the clinical practical value of 3D CE-MRA in diagnosis of arteriosclerosis obliterans in lower extremities . Methods Forty patients with arterial diseases of lower extremities were examined by 3D CE-MRA using a 1.5 T MR (Magneton Avanto,Siemens ) equipped with 32 receiving channels and integrated parallel acquisition techniques(iPAT), among which 24 were also examined by color Doppler ultrasound . Results Different degree of stricture and obturation were discovered in 37 cases of above 40 cases with clinically documente diseases of lower extremities. The positive rate of diagnosis was 92.5%(37/40).The coincidence rate with operation was 92.7% while diagnose color Dopper ultrasound was 88%. Conclusions CE-MRA poses particular adavantage on the pathology of atherosclerotic, which is a non-invasive, safe, reliable, and practical methods of examination, even this method is first choice.
