摘要
目的研究眼动脉起点周围结构的显微解剖,为经对侧手术入路治疗颈内动脉-眼动脉瘤提供解剖学依据。方法取25例(50侧)成人尸头标本,对眼动脉起点及其周围结构进行显微解剖观察测量。结果84%的眼动脉起源于颈内动脉(ICA)床突上段,其中81%起源于ICA上壁内侧三分之一。眼动脉起点均位于前床突尖前下方,起源于ICA床突上段的眼动脉中,41%起源于镰状韧带边缘的前下方,59%起源于镰状韧带边缘的后下方。结论经对侧入路能更好的暴露眼动脉起点,对侧入路适用于较小的向内侧突出而未破裂的眼动脉瘤。
Objective To study the microsurgical anatomy of the structures around the origin of ophthalmic artery to provide the microanatomic basis for contralateral surgical approach to carotid-ophthalmic aneurysm. Methods The origin of ophthalmic artery and adjacent structures were observed on 25 adult cadaveric head specimens by microanatomic methods. Results The ophthalmic artery originated from the supraclinoid segment of internal carotid artery ( ICA ) was 84%, and 81% of them arose from the medial third of superior surface of ICA. Almost all of the ophthalmic arteries originated anteroinferior to the tip of anterior clinoid process, ophthalmic arteries originated from the srpraclinoid segment of ICA anteroinferior to the edge of the falciform fold were 41%, and posteroinferior 59%. Conclusions It is easy to expose the ophthalmic artery via contralateral approach which is recommended for small, medially extended and silent carotid-ophthalmic aneurysm.
