肾脏功能不同程度损害时尿中几种微量蛋白与尿酶变化分析 被引量:6

摘要 目的探讨肾脏在不同程度损伤时最有诊断价值和监控作用的检测指标变化特点。方法对84例不同肾脏功能损伤患者及30例健康对照者进行尿蛋白定量分析和尿微量白蛋白(mAlb)、转铁蛋白(UTf)、a1-微球蛋白(a1-MG)、β2-微球蛋白(β2-MG)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)检测,并对检测结果进行统计、对比、分析。结果当肾脏功能有不同程度损伤时,mAlb、UTf、a1-MG、β2-MG、NAG均有不同程度的变化,且与健康对照组比较差异有统计学意义(P〈0.01);随着病情加重,其变化较明显(P〈0.01)。结论NAG、UTf、a1-MG、β2-MG、mAlb是肾脏功能早期损害的诊断指标,其检测水平的变化在肾脏损伤程度中较灵敏,对病情的诊断和监控有较积极的意义和作用。 To study the most valuable detection index to diagnose and monitor the degree of kidney functional lesion. Methods The levels of urinary microalbumin (mAlb), urinary transferrin (UTf), a1-microglobulin (a1-MG), β2-microglobulin (β2-MG) and N-acetyl-β-D-glucosaminidase (NAG) were detected in 84 patients with kidney function damage and 30 healthy controls, and the statistical analysis on the results was performed. Results The levels of mAlb, UTf, a1-MG, β2-MG and NAG changed differently when kidney function was damaged to different degree,and the differences had statistical significance compared as compared with those of control group (P〈 0.01). Along with the pathogenetic condition aggravating, these changes were more marked. Conclusion mAlb, UTf, a1-MG, β2-MG and NAG are sensitive early indices for diagnosis of kidney functional lesion, and play an important role in diagnosing and monitoring of different degree of kidney functional lesion.
