责任医生进家庭的实践与体会

Practice and Experience of Health Visit by Family Doctors

2005年杭州长庆、潮鸣社区卫生服务中心开展了责任医生进家庭的试点工作。通过组建责任医生团队、制定职责、划片分管、上门调查摸底、采用针对性服务内容与干预措施,责任医生得到了社区居民的认可。社区居民的满意率98%、责任医生的知晓率99%、慢性病患者管理率100%、健康教育自觉参与率86%。

Objective In 2005, in Changqing - Chaoming community health service center the pilot work on home visit by doctors was conducted. Through establishing community doctor＇s team, formulating responsibilities, designating area of home visit making field survey, and adopting aim - based service and intervention, the family doctors had been accepted by community residents. The satisfying rate of the residents was 98% ; the knowing rate of their serving residents by the doctors was 99% ; managed rate of chronic diseases patients was 100% and attending rate of health education by the residents was 86%.
