摘要
目的通过对阿维A酸治疗银屑病的随机对照试验研究进行系统评价,了解阿维A酸治疗银屑病的有效性。方法将观察应用阿维A酸治疗银屑病患者疗效的随机或半随机对照试验进行分析,将有效率作为观察指标。检索中英文文献应用Medline、Pubmed(1966-2004)、Ovid、The Cochrane library2004.4th、中国生物医学文献数据库(1978-2004)、Vip全文数据库(1994-2004)、解放军医学图书馆、国家科技图书文献中心的数据库进行电子检索结合手工检索国内的期刊杂志和药厂提供的资料。应用Jadad量表的质量标准进行文献评价,RevMan4.2版软件进行统计分析。用比值比(OR)和95%CI为疗效分析统计量。采用SPSS和SAS软件制作漏斗图分析潜在的发表偏倚。结果有4个阿维A酸治疗银屑病研究的有效率的按亚组分类后,组间无异质(χ2=1.40,P=0.71,I2=0%),用固定效应模型进行合并,结果显示应用治疗阿维A酸治疗银屑病的有效率与对照组差异有统计学意义[OR5.93,95%CI(2.79,12.57)]。结论阿维A酸治疗银屑病是有效的,其安全性还需进一步评价,今后随着高质量的报告出现,可以不断的完善本评价。
Objective To evaluate the therapeutic effect of acitretin by systematic review of the randomized or quasirandomized controlled trials in treating psoriasis. Methods This systematic review use Evidence-Based Medicine. Criteria for considering studies for this review are the randomized or quasi-randomized controlled trials of acitretin in patients with psoriasis from Medline, Pubmed（1996-2004）, Ovid, the Coehrane library2004.4^th, CBMdsic, Vip, mlpla, NSTL and some Chinese journals. Methodolgical quality assessment and data extraction assessment for methodological quality was using a modification of criterriagive in the Cochrane Reviewers, Handbook and the criteria of Jadad. Revman 4.2 was used for Meta-analysis. For these data ,odds ratio was used to calculate the summary estimate, applying 95% confidence intervals （CI）. Using SAS and SPSS for making funnel plot to analysis the bias. Results Four studies comparing acitretin with controlled group showed significantly more improvement. （OR=5.93, 95%CI 2.71-12.57） No heterogeneity was found among five studies（P-0.71）. Conclusion Acitretin is an effective treatment for psoriasis, but its safety need a further evaluation. With the high quality reports appeared, it would be poosible to improve this evaluation.
