乳腺癌改良根治下腹横行腹直肌肌皮瓣一期乳房再造之探讨

Using TRAM to reconstract breast at once after doing improving operation on breast cancer

目的:介绍下腹横行腹直肌肌皮瓣(TRAM)进行乳房再造的方法。方法:将下腹部TRAM皮瓣的安全界限分为四区。耻区设计横行皮肤切口,形成含腹壁上动静脉的腹直肌蒂,进行即时乳房再造。结果:应用该方法乳房再造9例,均为单蒂TRAM,乳房形态良好。结论:耻区TRAM皮瓣再造的乳房体积大,容易塑形,形态满意,且同时具有腹部减肥的优点。此方法满足肿瘤治疗和形体美容两方面要求,值得推广。

Objective ： Introducing a method of using TRAM to reconstract breast. Methods ： The safety limit of TRAM is usually divided to 4 section. We design a infestative skin cut to form the pedicel of M. Rectus abdominis including A＆V. Epigastrica superior to reconstruct breast. Results ：We using this method to reconstruct breast for 9 patients（they are all single pedicel）. And the shape of breast is good. Conclusion：The reconstructive breast with TRAM is bigger than normal and is easy to shape. Using this method can also cut the fat of abdoman. We think this method is fit for tumour theraphy and body shape and is worth to popularize.
