灰阶超声、彩色多普勒超声在早期乳腺癌诊断的应用价值 被引量:13

The role of gray-scale and color Doppler US in diagnosing early breast carcinoma

摘要 目的探讨灰阶超声、彩色多普勒超声在早期乳腺癌诊断中的应用价值。方法对209例女性患者(218个肿瘤)术前进行灰阶超声、彩色多普勒超声检查,观察乳腺肿块形态、边缘、内部回声及钙化、后方回声以及血流信号分布、测定血流参数。术后与病理结果对照分析。结果218个肿瘤手术病理诊断早期乳腺癌43个,良性肿瘤175个。灰阶超声诊断早期乳腺癌的敏感性,特异性,准确率分别为69.8%,77.1%,75.7%。灰阶超声与彩色多普勒超声相结合,其敏感性,特异性,准确率分别为83.7%,84.6%,84.4%。结论灰阶超声与彩色多普勒超声检查相结合可明显提高乳腺肿块超声诊断的准确率。 Objective To evaluate the diagnostic value of gray-scale and color Dopple scanning in early mammocarcinom. Methods Two hundred and nine patients who suspected suffering mammoneoplasm were examind, and 218 solid lesions of the shape, boundary, echogenicity, calcification, enhancing or decreasing of post lesion and color Doppler signals were observed before operation. Diagnosis was made in each case and compared with the pathological results. Results Forty three solid lesions were confirmed early breast carcinoma by pathology and 175 solid lesions were confirmed benign masses after operation. The sensitivity, specificity and accuracy of gray-scale sonography in diagnosing early breast carcinoma were 69.8%, 77.1%, 75.7% respectively. The sensitivity, specificity and accuracy of Color Doppler ultrasonography combined with gray-scale sonography were 83.7 %, 84.6 %, 84.4 % respectively. Conclusion The color Doppler parameters can obviously increase the diagnostic accuracy of breast masses if combined with gray-scale sonography features.
