MR弥散加权成像在鉴别前列腺良恶性组织中的价值 被引量:1

摘要 目的研究前列腺良恶性病灶的磁共振表观弥散系数(apparent diffusion coefficient,ADC)的差异。方法对健康对照27例、前列腺癌(PCa)患者25例行MR弥散加权成像(diffusion weighted imaging,DWI)。按B超引导穿刺活检操作医师在前列腺样图中标出的相应位置选出感兴趣区域(regions of interest,ROI),分别对癌灶和正常前列腺组织的ADC值进行测定,比较其差异是否存在统计学意义。结果健康对照组的ADC平均值(1.73±0.16)×10-3mm2/s;最低值(1.30±0.15)×10-3mm2/s;PCa组ADC平均值(0.81±0.12)×10-3mm2/s;最低值(0.58±0.10)×10-3mm2/s,分别通过t检验(P<0.001),两组间差异均存在统计学意义。结论ADC值能定量地反映前列腺组织信息,PCa病灶的ADC值明显低于正常组织。ADC值是鉴别前列腺良恶性病灶一个很有价值的可定量参数。 Objective To evaluate Ihe vahle of MR diffusion weighted imaging (DWI) and apparent diffusion coefficienl (ADC) in differential diagnosis of benign and malignant lesions of the prostate. Methods 27 healthy volunleers and 25 patients with prostate cancer (PCa) were examined by MR-DWI. Regions of interest (ROI) were drawn on the ADC map by referring to the urologist' s illustration of TRUS-guided biopsy sites. ADC values of cancerous tissue were compared to those of noncancerous tissue by statistical analysis. Results The average and minimum ADC was (1.73±0.16)×10^-3 mm^2/s and (1.30±0.15)×10^-3mm^2/s in healthy controls, and (0.81±0.12)×10^-3mm^2/s and (0.58±0.10)×10^-3mm^2/s in PCa patients, respectively. There was significant difference with a P value〈0.001 using a t-test. Conclusions ADC value was lower in cancerous tissue than that in healthy tissue of the prostate. ADC is an effective parameter in distinguishing malignant and benign proslale lesions.
