彩色室壁运动定量分析腹主动脉瘤壁节段运动

摘要 目的:运用彩色室壁运动(CK)定量分析正常腹主动脉壁及腹主动脉瘤的瘤壁运动。材料和方法:使用CK技术对20例正常青年男性(平均年龄24±2.3年,正常组)及20例老年男性(平均年龄65±3.4年,老年组)的腹主动脉壁运动和10例腹主动脉瘤患者(平均年龄65±5.2年,动脉瘤组)的动脉瘤壁运动进行观测和记录,并使用定量分析软件(ICK^(TM))对所得图像加以分析。结果:使用M型超声和CK测量正常组腹主动脉前后壁位移相似,无统计学差异。在正常人腹主动脉壁位移,后壁0.93±0.21mm,前壁1.42±0.24mm;老年人腹主动脉壁的位移,后壁0.91±0.12mm,前壁1.23±0.18mm。腹主动脉瘤患者的瘤壁位移,后壁:入口处0.31±0.22mm,最大直径处0.11±0.16mm,出口处0.22±0.13mm;前壁:人口处0.66±0.42mm,最大直径处0.21±0.18mm,出口处0.31±0.15mm。在横断面上,收缩期正常组腹主动脉的各节段是同时达到最大位移,而腹主动脉瘤的瘤壁运动各节段是不一致的。结论:使用CK和定量分析软件(ICK^(TM))观测腹主动脉和腹主动脉瘤壁节段运动是可行的,可以对瘤壁运动提供更多的信息。 Purpose: To make it clear whether it can be applied with color kinesis (CK) and "ICK^TM Quantitative Analysis System" in research of wall movement of infrarenal abdominal aorta and abdominal aortic aneurysm(AAA) . Materials and Methods : Twenty young men (mean age 24 ± 2.3y, normal group, N), twenty old men(mean age 652 3.4y, old group, O) and ten patients(mean age 65±5.2y, aneurysm group, A) with infrarenal abdominal aortic aneurysm were involved. The wall movement of aorta or AAA was observed by color kinesis and the image was analyzed with "ICKTM quantitative analysis system". Results: There is no significant different about displacement of anterior or posterior wall which was measured by M - mode or CK in group N. The displacement of abdominal aortic anterior wall is 1.42 ± 0.24mm and 1.16 ±0.18mm in group N and O. The displacement of the abdominal aortic posterior wall is 0.93 ± 0.21mm and 0.91 ± 0.12mm in group N and O. The displacement of AAA wall is difference in difference locations. The anterior wall is 0.66 ± 0.42mm, 0.21 ± 0.18mm, 0.31 ± 0.22mm and the posterior wall is 0.31 ± 0.22mm, 0.11 ± 0.16mm, 0.22 ± 0.13mm in inlet, the maximum diameter and outlet respectively. In cross - section, the movement of aortic wall in difference segment is concordant and symmetrical basically in group N , but the characteristic is vanished in aneurysmatic wall. Conclusion: CK was a sensitive method to detect regional wall movement of abdominal aorta and AAA. It is feasible to observe wall movement of AAA with color kinesis (CK) and "ICK^TM Quantitative Analysis System".
