跟骨骨折53例治疗体会

Experience on 53 cases of patients with fractures of the calcaneus

目的:观察手法整复石膏固定跟骨反弹器固定术及切开复位“AO”钛板固定术治疗跟骨骨折疗效。方法:53例患者中,按Rowe分型Ⅰ～Ⅲ型患者采用手法复位石膏固定治疗;Ⅳ、Ⅴ型单纯塌陷的舌形骨折患者采用透视下手法整复跟骨反弹器外固定术治疗;Ⅳ、Ⅴ型严重粉碎性骨折患者采用切开复位“AO”钛板内固定术治疗。结果:53例患者术后常规摄跟骨正轴位X线片示跟骨结节角恢复正常者48例,10°～20°5例。愈合时间平均为11w,6个月～12个月拆除内固定。结论:手法整复石膏固定跟骨反弹器固定术及切开复位“AO”钛板固定术治疗跟骨骨折疗效显著,可进一步推广应用。

Objective：To observe the therapeutic effect of gypsum fixation and rebound device for fixation as well as open reduction of ＂AO＂plate fixation in the treatment of fractures of calcaneus. Methods.Among the 53 cases, types of Ⅰ～Ⅲ were respectively given the manipulation and gypsum fixed therapy; patients of Ⅳ、Ⅴ-type with collapse of the tongue were under the external rebound fixation of calcaneus through fluoroscope device for the treatment; patients with Ⅳ、Ⅴ with serious fracture were treated by＂AO＂internal titanium fixation. Results.After the treatment , conventional axial X-ray showed that calcaneal tubercle angle returned to normal in 48 patients among the 53 cases,with 5 cases of 10 ; ; 20 ;. The average healing time is 11 w,and removed the internal fixation within 6 -12 months. Conclusion： It can get significant effect to use gypsum fixation and rebound device for fixation as well as and open reduction of＂AO＂plate fixation and is worthy of being promoted further.
