“加血管法”容积重建CT脑血管造影的临床应用

摘要 探讨“加血管法”三维容积重建(VR)CT脑血管造影的成像方法,评价其临床应用价值。材料和方法分析37例脑CTA图像,观察分析常规VR技术和“加血管法”VR两种方法对正常血管和血管性病变的显示情况,并对照DSA和/或手术结果。结果37例中17例可见病变,包括动脉瘤11例,血管狭窄闭塞性病变4例,动静脉畸形2例有DSA或手术对照的18例中16例诊断正确,假阳性和假阴性各1例,诊断正确率88.9%。与常规VR相比,加血管法VRCTA显示脑血管分支清晰,走行自然,具有良好的对比度和锐利度,无重叠、没有无关信息干扰,小血管的显示更清晰,尤其对目标血管三维显示良好。但在统计学上对脑血管的显示与常规VR法比较无显著性差异(P>0.25)。结论颅脑CTA“加血管法”VR技术对正常血管和病理情况的显示优越,可加以推广。 Objective To investigate the method of add vascular volume rendering in the cerebral CT angiography and evaluated its clinical application. Methods Thirty- seven cerebral CT angiography using conventional VR method and add vascular method were analyzed retrospectively,two methods were compared in view normal vascular and vascular lesion,and controlled with DSA and/or operation result. Results In 37 cases 17 cases had lesion,included 11 aneurysm,steno-occlusion vascular in 4 cases,and 2 cases with AVM. The diagnosis accuracy was 88.9 % (16/18) compare to DSA and/or operation result, false positive in 1 ,and 1 false negative. Compared with conventional VR method,the vascular branch and small vascular were shown more clearly with add vascular method, no overlap and disturbance by other structure. But no significant difference in statistics between two methods. Conclusions The volume rendering method using add vascular was superior than conventional VR in manifestation of normal cerebral vascular and vessel disease, and can be used more extensive in clinical.
