PPH治疗痔病的远期临床疗效观察

Clinical Observation on the Long-term Therapeutic Effect of PPH in the Treatment of Hemorrhoids

为探讨吻合器痔上黏膜环切钉合术（PPH）治疗Ⅱ～Ⅳ度痔病的临床远期疗效，对行PPH治疗的539例Ⅱ～Ⅳ度痔病患者，比较患者术前和随访终点时的脱垂、出血、疼痛和便秘情况。随访时间24～49个月，平均随访时间30．7个月，只有极少数患者随访时有少量便血或轻微脱垂表现，其余恢复良好，复发率低。结果表明，PPH除了具有手术时间短、疼痛轻、创伤小、恢复快、近期疗效显著等优点外，中、远期疗效也很肯定，是治疗痔病的理想方法。

The objective of the study was to evaluate the long-term therapeutic effect of PPH in the treatment of hemorrhoids of Ⅱ-Ⅳ grade. The diseased conditions of prolapse, hemorrhage, pain and constipation both before PPH and at the follow-up end points were compared for 539 patients with hemorrhoids of Ⅱ-Ⅳ grade who had undergone PPH. The mean follow up time was 30.7 monthes（range 24-49 monthes）. It was found during follow up that most of the cases were well recovered with a low relapse rate,except that there existed slight hematochezia and mild prolapse in fewer patients. It is revealed that PPH is an optimal surgical option in the treatment of hemorrhoids,not only in the short-term therapeutical effect （shorter operative time, lighter pain, less surgical damage,quicker recovery,etc. ）but also in long-term therapeutic effect.
