口腔颌面部恶性淋巴瘤的临床特征及诊断

The clinical features and diagnosis of malignant lymphomas in oral and maxillofacial region

目的:进一步认识口腔颌面部恶性淋巴瘤的临床特征,提高其早期确诊率。方法:对1998年8月-2006年2月收住的9例恶性淋巴瘤患者的临床症状、体征及相关影像资料、诊断过程进行回顾性分析。结果:口腔颌面部恶性淋巴瘤有肿块型、炎症型、坏死型等类型,除口腔颌面部病损外,还可累及肝、脾、腹膜后及全身的淋巴结,CT检查缺乏特征性变化。全部病例最终均需经病检确诊,腹部B超及胸部正位片对临床分期有重要意义。结论:口腔颌面部恶性淋巴瘤临床表现多样且不典型,极易被误诊,对可疑病例应尽早行病检确诊。

Objective ：This study aims at further understanding the clinical features of malignant lymphomas in oral and maxillofacial region and increasing the early-stage definite diagnosis rate. Methods ..Nine malignant lymphoma cases were collected from the department of stomatology of our hospital during the period from August 1998 to February 2006,their clinical symptoms,physical signs,related radiological data and diagnostic process were analyzed retrospectively. Results ：Malignant lymphomas in oral and maxillofacial region were divided into bump type,inflammation type and necrosis type,etc. Besides the damages on the oral and maxillofacial region,the lymphoma can implicate liver,spleen,the lymph nodes of retroperitoneum and the whole body,characteristic changes can not be seen from CT examinations. All cases were diagnosed defnitely through pathological examination,the abdomen type-B ultrasonic examination and chest PA X-ray photographs are important for correct stage grouping. Conclusion;The clinical manifestations of malignant lymphomas in oral and maxillofacial region are multiple and atypical ,they can easily lead to erroneous diagnosis,so the suspicious cases should be diagnosed definitely through pathological examination as Soon as possible.
