白内障不同手术切口和术式术后角膜内皮细胞的变化

THE EFFECT OF DIFFERENT INCISIONS AND METHODS ON CORNEAL ENDOTHEAL CHANGES IN CATARACT SURGERY

目的 比较白内障手术不同的手术切口和术式术后角膜内皮细胞的变化。方法 应用TOPCONSP-2000P式角膜内皮显微镜，计数行透明角膜切口超声乳化法、巩膜隧道切口超声乳化法、小切口ECCE法手术白内障病人术前以及术后3、6、12个月角膜内皮细胞数量，计算细胞平均密度及细胞平均丢失率。结果 3种不同术式及切口术后角膜内皮细胞密度差异无显著性（F=0．62～0．99，P〉0．05），角膜内皮细胞的丢失率差异亦无显著性（χ^2=0．76～0．91，P〉0．05）。结论 白内障手术切口、术式与术后角膜内皮细胞丢失无直接关系。

Objective To compare the influence of different incisions and methods on histological changes of corneal endothelium in cataract surgery. Methods Patients with cataract were operated on by phacoemulsification with clear cornea incision, or scleral tunnel incision or extra capsul cataract extraction small incision. The density of corneal endothelium was measured via TOPCON SP 2000P specular microscopy preoperatively and 3, 6 and 12 months postoperatively. Results The difference of changes in the density of corneal endothelium were not significant among the three different incisions （F=0. 62-0. 99 ,P〉0.05）, and the difference of percentages of endothelium loss was not significantly either （χ^2 =0.76-0.91, P〉0.05）. Conclusion Cataract operation with different incisions and methods does not produce significant influence on the loss of corneal endothelial cells.
