慢性乙型肝炎患者血清HBV-DNA载量与肝组织病理的关系

The Relationship betwecn the HBV DNA Load and Clinical or Liver Pathology in Patients with Chronic Hepatitis B.

目的:探讨慢性乙型肝炎(CHB)患者血清HBV-DNA载量与临床及肝组织病理的关系。方法:测定70例CHB患者血清ALT、TBIL、HBV-DNA载量,同时行肝活体组织检查,对肝脏进行炎症分级和纤维分期。结果:(1)血清HBeAg阳性组HBV-DNA定量水平明显高于HBeAg阴性组,血清HBV-DNA载量与HBeAg的表达有显著相关性(t=3.98 P<0.001);(2)HBV-DNA载量与肝损害程度不存在明确的相关性(F=1.1 P>0.05);(3)慢性乙型肝炎血清HBV-DNA载量与肝功能ALT、TBIL不存在明确的相关性(P>0.05);(4)肝脏病理炎症分级G1、G2、G3、G4之间及纤维化分期S1、S2、S3、S4之间和HBV-DNA载量比较无统计学差异(P>0.05)。结论:血清HBV-DNA载量与HBeAg的表达有显著相关性,HBV-DNA载量与肝功能ALT、TBIL及肝损害程度没有必然的联系,与肝组织病理炎症分级及纤维化分期无明显相关性。

Objective：To investigate the relationship between the HBV DNA load and clinical or liver pathology in patients with chronic hepatitis B. Methods： Seventy the lael of serum ALT, bilirubin, HBV DNA load were detected and liver biopsy was done liver inflammation grade and fiber phases in 70 CHB patients. Results： （1） The HBV DNA level of HBeAg positive group was significantly higher to get than that of HBeAg-negative. The expression of serum HBeAg was significantly correlated with the HBV DNA load （t = 3.98,P〈0. 001） ; （2） There was no clear correlation between the HBV DNA load and the degree of liver damage （P〉0. 05） ; （3） There was no clear correlation between the HBV DNA load and serum ALT, bilirubin （P〉0.05 P〉0.05） ; （4） The HBV DNA load was not significant deviation between liver inflammation grade G1、G2、G3、G4 and fibrosis S1、S2、S3、S4 （P〉0. 05）. Conclusion： Serum HBV DNA load have significant correlation with the expression of HBeAg, but nothing to do with ALT, bilirubin, histopathological grading and staging of hepatic fibrosis.
