冠心病伴胰岛素抵抗与血浆血脂纤溶系统关系的研究

摘要 目的:探讨冠心病伴胰岛素抵抗与血浆血脂纤溶系统的关系。方法:将糖耐量正常的80例冠心病患者作为研究对象,测定空腹和2h血糖和胰岛素水平,分为胰岛素抵抗组(IR组)38例,和非胰岛素抵抗组(non-IR组)42例,全部病例计算胰岛素抵抗值(HOMA-IR)、并测定总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、纤维蛋白原(Fib)、纤溶酶原激活物(t-PA)、纤溶酶原激活物抑制物Ⅰ(PAI-1)活性,进行2组比较及胰岛素抵抗与单因素相关分析。结果:IR组空腹、2h胰岛素(FINS、PINS)、Fib、PAI-1较non-IR组显著升高(P〈0.01),HDL-C、t-PA明显降低(P〈0.01),HOMA-IR与TG、Fib、PAI-1呈正相关(r=0.52、0.58、0.56,P〈0.01)。与HDL-C、t-PA呈负相关(r=-0.48、-0.46,P〈0.01)。结论:冠心病患者伴胰岛素抵抗是导致血脂代谢紊乱及高凝状态的重要因素。
